Lightning Learning: Pre-Tibial Laceration
STOP!
Pre-tibial lacerations usually affects the older population.
This happens when skin over the shin becomes thin and more friable as we age. Therefore, a small injury can cause the skin to tear.
More common in females.
These wounds need careful management and are likely to take weeks to heal.
The patient should be assessed fully, not just their wound.
LOOK
The skin over the shin has a poor blood supply. This is important when closing the wound!
Steri-strips should be used instead of sutures. Sutures may tear the skin and compromise vascular supply.
The patient’s medical history and medications are relevant.
Peripheral vascular disease, diabetes and steroid use will affect wound healing. Anti-coagulation could cause a haematoma.
If there is skin loss then it will likely need a Plastics referral.
